Heart diseases prevention: what can we really do?

Science suggests a number of risk factors for heart diseases: age, gender, high blood pressure, hyperlipidemia, diabetes mellitus, tobacco smoking, processed meat consumption, excessive alcohol consumption, sugar consumption, family history, obesity, lack of physical activity, psychosocial factors, and air pollution.
In your opinion, what can we really do to prevent it?
